On vehicles with the affected engine the piston crown may develop a crack creating elevated crankcase pressure leading to a fracture of the valve cover. A fractured valve cover may allow oil to escape and in the presence of an ignition source increases the risk of a fire.

Fix: Install a crankcase pressure sensor and a pressure relief valve.

On some TRISTOP� actuators the chambers may not have been correctly assembled and the load from power spring and pressure is not fully taken. Under certain circumstances the parking side and the service sides of the actuator could separate and the actuator could lose functionality. Ultimately this could lead to a loss of braking functionality and/or brake failure. Critical applications are vehicles such as city buses or garbage collection vehicles (subject to a high parking brake frequency).

Fix: Identify and replace affected actuators.

Right hand rear brake actuator may contact axle casing when brake pads wear and calliper/actuator assembly move inboard. Actuator band clamp bolt may fracture resulting in separating of actuator service portion from main actuator body. If the concern occurs the stopping distance for the vehicle will increase.

Fix: Recall the vehicles that are likely to be affected and reposition the clamp band to ensure no risk of a foul condition remains. The clamp bolt will be examined for signs of contact and replaced if witness marks present.

A seat belt reinforcing bracket has been omitted during the retrofitting of kits to passenger seats on certain vehicles. As a consequence the anchorage may not meet the required standards.

It is possible that during the manufacturing test of the Hand Brake Valve that damage occurred to the handle locker. It is then feasible that the handle locker could come to rest in an unsecured position which could subsequently allow the handle to move to the released/drive position and result in a vehicle roll-away.

Fix: Recall all effected unit and check for damage. If found to be damaged replace the hand brake valve with a new quality assured item.

It has been identified that due to overheating of the brake disc failure of the adjacent brake may occur resulting in brake loss.

Fix: Recalled vehicles will be fitted with a brake hose re-route kit which positions the hose further from the brake disc thus ensuring that in the event of a brake overheat the hose/s are not affected.

It has been identified that the possibility exists that the footvalve piston may become dislodged. Should this occur the service brakes may become inoperative.

Fix: Recalled vehicles will have the service brake footvalve replaced.

It has been identified that the possibility exists when the rear brake pads are close to the end of their service life that the brake caliper may contact the chassis frame.

Fix: Recalled vehicles will have the rear bump stop bracket re-profiled thus providing additional clearance to accommodate maximum caliper travel.

It has been identified that the steering column universal joint may make rubbing contact with the column's outer casing. It is not thought that this will cause the steering to lock up.

Fix: Recalled vehicles will have a small part of the column retaining lug removed.
