  19. NRM · rear door may open without warning

    5 Dec 2016 · R/2016/293

It has been identified that the driver can inadvertently circumvent the correct operating procedure for the closing/opening of the rear plug and slide door. If the door operating sequence has been overridden then when the vehicle moves off from stationary the door can reopen a number of seconds after the vehicle has started to move off following the release of the park (halt) brake. Should this occur then the park (halt) brake will apply causing the vehicle to suddenly stop. The interlock will operate and the door will not reopen if the vehicle has exceeded 5 mph.

Fix: Recall all affected vehicles to upgrade the Kibes software.

  23. STREETDECK · directional control may be lost

    9 Mar 2016 · R/2016/044

Bolts (identified by CFL marking on head) connecting the lower wishbone assemblies to the vehicle chassis may be out of specification and at risk of failure. The failure of a single bolt is expected to have no adverse effect on the handling or integrity of the vehicle. A secondary failure mode on more than one bolt is likely to result in an unstable vehicle that will be difficult to control and will not handle in a normal way. The risk is that the driver could lose control of the vehicle.

Fix: Recall the vehicles that are likely to be affected and where a lower wishbone assembly is found to besecured with bolts marked CFL the bolts will be replaced with a bolt manufactured by a different supplier. All eight bolts in the lower wishbone assemblies irrespective of the manufacturer of the originally fitted bolt will be replaced when one or more bolts with the CFL marking are replaced.

Due to a software error in one of the vehicle's ECU's it is possible that if a passenger or occupant operates the front service door emergency controls (interior and exterior) when the vehicle is travelling in excess of 5km/hour then the front service door will function and open. This will only occur if the brake pedal is being pressed at the same time as the emergency control is operated. This could result in a passenger or occupant falling from the vehicle.
